YOUTH SPORTS REGISTRATION FORMS

• Family Agreement to Participate
  Each family must fill out a "Family Agreement to Participate" form with all correct information. This will only need to be done once.  If you have not participated in a Tremonton Parks and Recreation Program since the fall of 2008 then you will need to fill out a "Family Agreement to Participate".  Please list all Parent and children the first time you fill it out, even if they are not currently participating in anything. You may print this out and fill it in at home and bring it with you to registration to save you time. Thank You!
  • If you are sure you have filled out the "Family Agreement to participate", then you only have to fill out one of the following registration forms for each activity: (If you're not sure then fill out the Family Agreement to participate to be safe.)

• Candy Cane Hunt - No registration form. Registration is taken at the event.
• Hershey Track & Field - No registration form. Registration is taken at the event.
• Holiday House Decorating Contest Entry Form
• Jr. Jazz Fun Shot - No registration form. Registration is taken at the event.
• Jr. Olympic Skills Competition - No registration form. Registration is taken at the event.
• Kids Picture Contest - Enter a 8.5 x 11 Christmas themed drawing with the child's name, age, and address on the back.
• Movies in The Park - Free event for everyone, no registration required.
• NFL Punt, Pass, and Kick - No registration form. Registration is taken at the event.
