REGISTRATION
FORMS
All registrations are taken at the Tremonton City Offices 102 S. Tremont St.
**To save time at the City Office, please print off the appropriate registration form, fill it out and bring it in to register.
**If your child has never played with the city before please print off and fill out the Agreement to Participate form.
**For the Adult Activities please print off and fill out the Adult Activities Agreement to Participate form.
** A $5 late fee will be assessed on all late registrations. Late registrations are open for one week after the last day of registrations.
**Special requests are NOT guaranteed. Please see Policies & Procedures.
- Family Agreement to Participate
Each family must fill out a "Family Agreement to Participate" form with all correct information. This will only need to be done once. If you have not participated in a Tremonton Parks and Recreation Program since the fall of 2008 then you will need to fill out a "Family Agreement to Participate". Please list all Parents and children the first time you fill it out, even if they are not currently participating in anything. You may print this out and fill it in at home and bring it with you to save time. By providing a current e-mail address we will send you reminders of deadlines and registration dates. These e-mail addresses are only for our use and will not be used by anyone other than the sports and recreation office. Thank You!
